Modifiable Risk Assessment Template designed to be simple and easy to use, whilst still ensuring a through assessment through the use of an easy to use risk matrix (likelihood/consequence) and hierarchy of controls (most effective to least effective) based on the ESIRAP principles of:

E = Elimination Remove the hazard completely from the workplace or activity

S = Substitution Replace a hazard with a less dangerous one

I = Isolation Separate people from the  hazard

R = Redesign/Engineering Make a work process or activity safer

A = Administration Putting safety rules, signage or training in place

P = Personal Protective Equipment- Protect the worker through safety equipment
