Modifiable Payroll Deduction Form to clearly document the withholding of post-tax deductions from an employees pay.  This form is suitable for withholding extra tax, additional superannuation payments and for repayment of amounts to the employer (i.e. an account payment). This form allows the option for the payment to be paid weekly or as a once-off payment for a ongoing or specific time period.
